Patient Information

  • Male, 74 years old
  • Slurred speech and limb weakness for 6 hours.
  • Muscle strength: Right limbs – Grade 3, Left limbs – Normal
  • NIHSS score: 13 points

Preoperative Angiography

Preoperative Cranial CT: Multiple cerebral ischemic and infarct lesions.

Cranial DWI: Acute infarction in the left thalamus.

Preoperative Diagnosis

  • Acute cerebral infarction
  • Grade 3 hypertension (high risk)
  • Sequelae of prior cerebral infarction

Devices

  • 5F Zenith Aspiration Catheter
  • 6F Transradial Sheath
  • Mach1 6F Guiding Catheter

Procedure

Angiography via the right vertebral artery revealed basilar artery occlusion.

Right radial artery access was established. A pigtail catheter assisted in advancing the guiding catheter into the right vertebral artery. A 5F Zenith Aspiration Catheter was advanced to the occlusion site, and thrombus was successfully aspirated.

Postoperative angiography showed good vessel recanalization and restored blood flow.

Postoperative Angiography

Immediate postoperative cranial CT revealed no secondary hemorrhage.

1-day postoperative follow-up cranial CT showed stable intracranial condition.

Physical examination: Right limbs – Grade 3+, Left limbs – Normal.

NIHSS score: 7 points
